 Mine Registration, Reporting and Safeguarding Forms

 

PLEASE READ - IMPORTANT INSTRUCTIONS FOR DOWNLOADING/SAVING FORMS - PLEASE READ

The interactive pdf forms can be downloaded to your computer, completed electronically, and filed via email (or mailed or faxed):


1. Right click on the link "interactive pdf" and select "Save target as" from the menu.
2. Save the interactive pdf to your computer.

All active mining, milling and smelting operations (with the exception of those on Indian lands) must be registered using Form 1 (interactive pdf) and the information kept current using Form 2 (interactive pdf). Note that Form 1 has a second page that must be completed with mine location information.

Once registered, annual report forms 3-11 (as appropriate, listed and available below) are mailed to operators in early April for submission to MMD by April 30th. These forms provide statistical information for the previous calendar year. To be in compliance, operators need only to respond to the mail out, but may wish to review the information being requested by viewing the appropriate form(s).

MMD must be made aware of all temporary or permanent suspensions of an operation when an office with a responsible person present at all times is not maintained at the site. This notification is provided through submittal of Form 12 (interactive pdf). Submittal of that form results in an onsite safeguarding inspection (operators need not be present) where staff determines whether the closure is consistent with the Safeguarding Guidelines.
